What Determines Female Fertility?

Female fertility is influenced by a complex interplay of biological, lifestyle, and environmental factors. Unlike male fertility which continuously produces new sperm, women are born with all the eggs they will ever have. This finite ovarian reserve declines with age — making age the single strongest predictor of female fertility.

Key Fertility Factors

  • Age: Peak fertility in mid-20s; significant decline after 35; accelerated decline after 37
  • Ovarian Reserve: Measured by AMH and antral follicle count — indicates remaining egg supply
  • Cycle Regularity: Regular ovulatory cycles indicate healthy hormonal function
  • Body Weight: Both under and overweight BMI disrupts ovulation via estrogen imbalance
  • Smoking: Damages eggs, reduces ovarian reserve, and accelerates menopause onset by 1–4 years
  • Stress: Chronic stress elevates cortisol, suppressing GnRH and disrupting ovulation
  • Thyroid Function: Both hypo and hyperthyroidism impair fertility
When should I get fertility testing?
Seek fertility evaluation after 12 months of trying (under 35) or 6 months (35+). Earlier evaluation is appropriate with PCOS, irregular cycles, endometriosis, prior pelvic infections, or multiple miscarriages. Basic tests include cycle day 3 FSH, estradiol, and AMH for ovarian reserve.
