Folic acid is the most critical supplement for pregnancy. It is essential for the proper closure of the neural tube — the structure that becomes your baby's brain and spinal cord — which happens in the first 28 days after conception, often before you even know you're pregnant. Folic acid supplementation reduces the risk of neural tube defects (spina bifida, anencephaly) by up to 70%. Women with the MTHFR gene variant should use methylfolate (the active, pre-converted form).

400–800 mcg folic acid daily. Women with previous neural tube defect pregnancies or MTHFR variant: 5mg daily (prescription dose — consult doctor).
